Shoulder pain that won’t go away is one of the most frustrating experiences for a pitcher. Often, it’s brushed off as “fatigue,” but in many cases, the underlying issue is a SLAP tear — a tear in the top portion of the shoulder labrum where the biceps tendon anchors into the socket.

A SLAP tear (Superior Labrum Anterior-Posterior) disrupts the shoulder’s passive stability. The labrum acts like a suction cup, deepening the socket to keep the ball of the shoulder joint centered during motion. When it’s torn, the shoulder loses that stability, putting far more demand on the rotator cuff and scapular stabilizers — muscles responsible for dynamic control during high-velocity pitching.

Over time, this imbalance can cause further irritation to the biceps tendon or supraspinatus, leading to pain, tightness, and a loss of throwing power.

The 4 Phases of SLAP Tear Rehab

1️⃣ Acute Phase – Calm It Down & Protect the Tissue
The first step is reducing inflammation and protecting the healing tissue. Gentle range-of-motion drills, soft tissue work, and pain modulation techniques dominate this phase.

2️⃣ Strength Phase – Rebuild Stability & Control
Once pain decreases, we shift toward rotator cuff and scapular strengthening. Because the labrum can no longer provide the same passive stability, your muscles must take on that role. We also assess the hips and thoracic spine, since dysfunction there can overload the shoulder.

3️⃣ Advanced Strength & Power Phase – Prepare for Throwing
This is where we introduce plyometric progressions, controlled overhead loading, and advanced scapular control drills to recondition the arm for explosive motion.

4️⃣ Return-to-Throwing Phase – Build Velocity Safely
A structured throwing program gradually rebuilds velocity, volume, and tolerance. The focus is on smooth mechanics, incremental loading, and proper recovery between sessions.


Objective Return-to-Throw Criteria

Before a pitcher can safely throw again, several key benchmarks must be met:

  • Total Shoulder Range of Motion within 5° bilaterally

  • Cross-body adduction of at least 90°

  • External:Internal rotation strength ratio of ≥0.85:1

Only when these metrics are met should a pitcher begin a graduated throwing program — starting with light catch play and progressing through flat-grounds, bullpens, live BP, and ultimately game action.


Long-Term Arm Care Maintenance

Even after a full return, the work isn’t done. Ongoing rotator cuff strength and endurance remain non-negotiable. Integrate heavy external rotation, rhythmic stabilizations, crawling variations, and scapular control drills throughout the in-season to keep your shoulder healthy and durable.

Think of arm care as your maintenance plan — like changing the oil in your car before problems arise.
